Aya Healthcare is seeking Board Certified/Board Eligible primary care physicians for flexible, part-time locum tenens in-home health evaluations with Signify Health across Pennsylvania (float pool, Philadelphia area).
• Conduct pre-scheduled in-home health evaluations for health plan members: medication and medical history review plus physical evaluation
• About 6-9 visits per day with a pre-populated, patient-specific form for each evaluation; no prescribing, diagnosing or changes to treatment
• Flexible scheduling Monday-Sunday, 7A-7P; no nights or weekends required
• Regions include Altoona, Bradford, Erie, Harrisburg, Sunbury and Williamsport areas
• Start as soon as credentialed; travel logistics may be provided depending on eligibility

Job Responsibilities
• Review medications and medical history and complete a physical evaluation in the member's home
• Complete the provided patient-specific form for each evaluation; collect labs if required
• Manage your own schedule with Signify (7A-7P, Monday-Sunday); no prescribing or diagnosing
Qualifications
• Active Pennsylvania license required; clinical work within the past 2 years
• Submit 3 regions of interest
• DEA not required
• Must be located in the U.S.; orientation modules (10-12 hours) required
