Launched in 2022, At Home Harmony is a fast-growing home-based medical and pharmacy practice in Richmond, VA, caring for older adults with complex chronic conditions. Our team of 20 practitioners and more than 30 clinical support staff uses Medicare’s comprehensive care programs to deliver coordinated, high-quality care where seniors are most comfortable: at home.
In this clinical leadership role, you’ll lead and support a team of 3–4 experienced NPs and PAs, extending your expertise across a much larger patient population. The position is primarily office-based, with limited patient visits, and focuses on quality outcomes rather than patient volume.
Job Responsibilities
Review complex cases and develop collaborative treatment plans
Conduct limited patient visits when physician-level care is needed
Take part in interdisciplinary care coordination and quality initiatives
Use innovative technology, including AI-assisted documentation and remote patient monitoring
Help shape clinical care and protocols within a growing, progressive practiceMD or DO
Qualifications
At least 3 years of experience in Family Medicine, Internal Medicine or Geriatrics
Active, unrestricted medical license and DEA registration in good standing
Home-based care experience is a plus, but not required
Not Visa Eligible
Working Hours
No evenings, weekends, holidays or hospital rounds
Full-time preferred; 0.8 FTE considered
Primarily office-based, with limited home visits as needed

Collaborative leadership and mentorship of APPs
Clear, compassionate communication with patients, families and care teams
Interdisciplinary care coordination
Comfort with technology, including EMR, AI-assisted documentation and remote patient monitoring
A genuine passion for geriatric and home-based care
