Aya Healthcare is seeking Primary Care physicians for flexible in-home health evaluation work with Signify Health's Central Michigan float pool (Lansing, MI area). Start as soon as credentialed; multiple openings.
- Pre-scheduled in-home evaluations for Medicare Advantage, Medicaid and select commercial plan members
- Flexible schedule Monday-Sunday 7a-7p; no nights or weekends required; no on-call
- Average 6-9 visits per day; pre-populated patient forms provided; no prescribing
- Travel logistics available depending on eligibility
Job Responsibilities
• Complete medication and medical history review and physical evaluation using a pre-populated form (no prescribing, diagnosing or treatment changes)
• Average 6-9 visits per day; may collect labs
Qualifications
• Active Michigan license; DEA not required
• Minimum 60 hours of availability per month
• Provide 3 counties of interest at submittal; in-country providers only
