Job description
We are seeking a dedicated Family Medicine Physician to provide comprehensive, patient-centered primary care to individuals and families across all stages of life. The physician will assess, diagnose, and manage acute and chronic medical conditions while delivering preventive care, health screenings, immunizations, and patient education. The role emphasizes evidence-based medicine, care coordination, culturally responsive care, and collaboration with an interdisciplinary healthcare team.
Job Responsibilities
Evaluate and manage acute illnesses, chronic conditions, and complex medical needs.
Develop individualized treatment plans and coordinate ongoing patient care.
Make appropriate referrals to specialists, internal clinical services, and external community resources.
Collaborate with physicians, advanced practice providers, medical assistants, and other healthcare professionals to ensure coordinated patient care.
Provide clinical consultation and guidance to physicians and mid-level providers as needed.
Maintain accurate, timely, and complete patient documentation in the Electronic Health Record (EHR), preferably EPIC.
Follow established clinical protocols, evidence-based guidelines, and applicable regulatory requirements.
Participate in continuous quality improvement (CQI) initiatives and community health programs.
Address medical, behavioral, and social determinants of health through coordinated, patient-centered care.
Maintain professional communication with patients, families, and colleagues while promoting a respectful and inclusive clinical environment.
Utilize electronic documentation tools, including AI-assisted charting technology, when appropriate.
Qualifications
Current Drug Enforcement Administration (DEA) registration for prescribing controlled substances, as applicable.
Current Pediatric Advanced Life Support (PALS) certification.
Board-certified or board-eligible in Family Medicine.
Ability to obtain and maintain hospital privileges.
Strong clinical assessment, diagnostic, treatment planning, and care coordination skills.
Excellent written and verbal communication, interpersonal, and organizational abilities.
Proficiency with electronic health records and Microsoft Office applications.
Ability to work effectively within a multidisciplinary care team.
Commitment to patient-centered care, cultural responsiveness, health equity, and trauma-informed practices.
Must meet applicable federal eligibility requirements for employment with federally funded healthcare organizations.
Medical Spanish proficiency is highly preferred.
