Client is seeking a compassionate and mission-driven Family Medicine Physician (MD/DO) to join its growing primary care team in Vandergrift, Pennsylvania.

As a Federally Qualified Health Center (FQHC), Client is dedicated to providing high-quality, comprehensive, and accessible healthcare to underserved and underinsured individuals and families throughout the community.

The Family Medicine Physician will provide comprehensive primary care to patients across the lifespan while emphasizing continuity of care, preventive medicine, chronic disease management, and patient education. The physician will work collaboratively with an interdisciplinary healthcare team to improve patient outcomes and advance Client's mission of delivering accessible, patient-centered care.

The Family Medicine Physician is responsible for diagnosing and treating acute and chronic conditions, developing individualized treatment plans, coordinating specialty care, and promoting preventive health. The physician will also contribute to clinical leadership, quality improvement, staff development, and community outreach initiatives.

Job Responsibilities

Key Responsibilities:
Patient Care:
Provide comprehensive family medicine and primary care services to patients of all ages.
Obtain thorough medical histories, including chief complaint, history of present illness, past medical history, family history, social history, and review of systems.
Perform comprehensive physical examinations using appropriate clinical assessment techniques.
Diagnose, treat, and manage acute and chronic medical conditions.
Develop individualized treatment plans incorporating:
Patient education and counseling
Medications
Diagnostic testing
Preventive care
Appropriate follow-up
Order, interpret, and review laboratory tests, diagnostic studies, and imaging.
Maintain accurate, timely, and complete documentation in the Electronic Health Record (EHR).
Coordinate referrals to specialists and community resources when appropriate.
Provide follow-up care to assess treatment effectiveness and monitor disease progression.
Participate in after-hours call coverage and emergency patient care as assigned.

Clinical Leadership:
Help coordinate and maintain continuity of patient care across the organization.
Collaborate with Advanced Practice Providers (APPs), including providing consultation, supervision, and medical record review when required.
Authorize clinical protocols used by Advanced Practice Providers.
Assist with developing, implementing, and reviewing clinical policies and procedures.
Support compliance with applicable regulatory and accreditation standards.
Provide oversight of the CLIA-waived clinical laboratory in accordance with applicable regulations.
Participate in Quality Improvement (QI), Quality Assurance (QA), and clinical performance initiatives.
Assist in monitoring clinical quality indicators and organizational healthcare goals.
Participate in peer review activities and medical staff meetings.

Community Engagement & Education:
Provide health education to patients and families through individual and group educational opportunities.
Collaborate with community agencies and healthcare partners to improve access to care and patient outcomes.
Participate in continuing medical education and professional development.
Support the education, mentoring, and professional development of clinical staff and healthcare providers.
Represent Client at community outreach events and organizational activities.

Qualifications

Required Qualifications:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
Current Pennsylvania Medical License, or ability to obtain licensure prior to employment.
Board Certified or Board Eligible in Family Medicine.
Current DEA Registration, or ability to obtain.
Current BLS Certification.
Strong EHR and clinical documentation skills.
Excellent communication and interpersonal skills.
Demonstrated commitment to compassionate, patient-centered care.

Preferred Qualifications:
At least 1 year of experience in Family Medicine or Primary Care.
Previous experience working in a Client Center or Federally Qualified Health Center (FQHC).
Experience caring for medically underserved or underinsured populations.
