- Work Dates Needed: October 1 - for the month or up to 3 months long
- Preferred Schedule: M-F: 8AM-5PM
- Worksite Setting: hospital-owned outpatient facility
- Scope of Work: All Ages seen, 18 - 22 a day & dedicated MA
- Established Group: Family Medicine Physicians (1.2 FTE); NP (1 FTE); PA (1 FTE)
- Licenses, Certifications, Requirements: CA license, DEA, BC or BE in FP, no malpractice cases, must have 3 MD references that the provider has worked with for a min. of 1 year.
