Position Summary

We are seeking a compassionate and patient-focused Primary Care Physician (PCP) to provide comprehensive care to a senior/geriatric patient population. The role offers a collaborative, team-based environment focused on coordinated care, quality outcomes, and an exceptional patient experience.

Key Responsibilities

· Provide comprehensive primary care to senior and geriatric patients.

· Evaluate, diagnose, and treat patients according to established standards of care.

· Manage a patient panel of approximately 500 or fewer patients.

· See approximately 8-13 patients per day.

· Participate in team huddles and collaborative care planning.

· Maintain continuity of care and a high-quality patient experience.

· Complete accurate and timely EMR documentation.

· Collaborate with multidisciplinary care teams to support patient outcomes.

· Review quality outcomes and medical records with leadership.

Qualifications

· MD or DO from an accredited institution.

· Active, unrestricted Alabama medical license.

· Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.

· Active DEA license.

· Medicare and Medicaid provider numbers.
