Opportunity:
Physician - Family Medicine - Geriatric Medicine, Internal Medicine - Geriatric Medicine
Job Details:
Outpatient, clinic-only position with no call requirementsMonday through Friday schedule, with consideration for 3-4 days per week
Shift hours are 8:00 AM to 5:00 PM with 8 guaranteed hours per day
Minimum of 2 weeks per month required, with full-time preferred
See 4-6 patients per shift, serving exclusively adults over the age of 55 through a PACE (Program of All Inclusive Care for the Elderly) program
Provide both in-clinic visits and community-based visits at patient residences, including assisted living facilities, skilled nursing facilities, and personal homes
Off-site visits involve driving to patient locations and are typically accompanied by a nurse from the care team
Work alongside an 11-person interdisciplinary team with most patient management coordinated through the EMR
Geriatric experience strongly preferred as the majority of the patient population is elderly
Board certification or true board eligibility required
Active state license and DEA required
BLS certification required
