Practice Setting
Providers will work in an outpatient primary care clinic dedicated to serving primarily adults and geriatric patients ages 55 and older. The clinic focuses on comprehensive health management, preventive care, chronic disease treatment, and quality-driven healthcare initiatives.
The value-based care model allows physicians to spend additional time addressing patient needs, coordinating care, and improving long-term health outcomes.
Clinical Responsibilities
- Perform comprehensive health assessments and annual wellness visits
- Diagnose and manage acute and chronic medical conditions
- Provide preventive care and patient education
- Manage medically complex and high-risk patient populations
- Identify and close care gaps to improve quality outcomes
- Address HEDIS and quality metric initiatives
- Support Medicare Advantage and value-based care objectives
- Manage minor behavioral health concerns as needed
- Coordinate care with specialists and ancillary services
- Document patient encounters within Athena EMR
Schedule
40 Hours Per Week
- Monday: 8:00 AM - 5:00 PM
- Tuesday: 8:00 AM - 5:00 PM
- Wednesday: 8:00 AM - 5:00 PM
- Thursday: 8:00 AM - 5:00 PM
Additional Details
- One-hour unpaid lunch
- No weekends
- No call responsibilities
- No travel between clinics
- Credentialing typically completed in approximately two weeks
- Assignment begins upon credentialing completion
- Strong likelihood of extension beyond initial contract term

Qualifications
Required
- MD or DO
- Board-Certified Family Medicine or Internal Medicine Physician (ABMS or AOA recognized boards)
- Active Tennessee Medical License
- Active Tennessee DEA Registration
- BLS Certification
- Minimum one year of outpatient primary care experience
- Experience treating adult and geriatric patient populations
- Familiarity with Value-Based Care models
- Experience with HEDIS measures, STARS programs, or care gap closures
- Comfortable managing minor behavioral health concerns

- Medicare Advantage experience
- Accountable Care Organization (ACO) experience
- Managed care experience
- FQHC experience
- Athena EMR experience
- Geriatric-focused primary care experience
