Primary Care Physician – Southbury, Connecticut

The organization is seeking a Board Certified/Board Eligible Primary Care Physician to join a well-established, collegial medical team in Southbury, CT.Family Medicine and Internal Medicine physicians are encouraged to apply. New graduates and recent residents are welcome!
Essential Position Responsibilities
- Provide comprehensive primary care to adult patients across a broad range of medical needs.
- Conduct thorough patient evaluations and develop individualized treatment plans.
- Diagnose and manage acute illnesses and chronic medical conditions.
- Order, review, and interpret appropriate diagnostic testing.
- Prescribe medications and treatments based on evidence-based clinical guidelines.
- Provide preventive care, health screenings, wellness counseling, and patient education.
- Establish strong, long-term relationships with patients through continuity of care.
- Collaborate with specialists, advanced practice providers, nurses, and other healthcare professionals to ensure coordinated care.
- Maintain accurate, timely, and compliant documentation within the electronic medical record.
- Participate in quality improvement, population health, and other organizational initiatives.
Qualifications
- Education: MD or DO degree from an accredited medical school.
- Specialty: Family Medicine or Internal Medicine.
- Licensure & Certification: Board Certified/Board Eligible; eligible for Connecticut medical licensure.
- Experience: Previous clinical experience preferred, but new graduates and recent residents are welcome to apply.
