Join a Patient-Centered Medical Home Providing Whole-Person Care
A patient-centered medical home in Marion, Ohio is seeking a compassionate Family Medicine Physician to join its integrated care team. This is an opportunity to provide meaningful, relationship-based care in a collaborative environment where primary care, dental, behavioral health, nursing, and care coordination work together to support patients across the lifespan.

Model: Integrated, team-based care
Focus: Preventive care, chronic disease management, acute care & population health
Board Certification: Family Medicine required; Family Medicine Obstetrics preferred/required per posting
Licensure: Active or eligible Ohio medical license
DEA: Required
What You’ll Do
Provide comprehensive full-scope Family Medicine for patients across the lifespan
Perform preventive, acute, and chronic care visits
Evaluate patients, conduct physical examinations, and order/interpret diagnostic testing
Develop evidence-based treatment plans and monitor outcomes
Manage same-day visits and acute outpatient needs
Coordinate specialty referrals and collaborate with other providers
Maintain accurate and timely EHR documentation
Partner with dental, behavioral health, nursing, and care coordination teams
Participate in quality improvement and population health initiatives
Educate patients on prevention, wellness, and disease management
Support community health initiatives consistent with the medical home model
Qualifications
MD or DO from an accredited medical school
Completion of an accredited Family Medicine residency
Board Certified or Board Eligible in Family Medicine
Family Medicine Obstetrics certification/experience as required by the posting
Active or eligible for an unrestricted Ohio medical license
DEA registration
Strong full-spectrum Family Medicine skills
Ability to manage acute and same-day outpatient needs
Excellent communication and teamwork skills
EHR proficiency and familiarity with data-driven clinical care
Interest or experience in community health and underserved populations
Commitment to culturally responsive, trauma-informed, patient-centered care
