Post-Acute / SNF Rounding - MD/DO- Minnesota

Aeris Medical Group is seeking a Locum Tenens Physician for a short-term post-acute rounding assignment in Minnesota!

• Coverage Dates: October 12–15 & October 19–20, 2026
• Only 6 Rounding Days
• Approximately 19–24 visits per day
• Approximately 125 total visits across the assignment
• Post-Acute / Skilled Nursing Facility Setting
• Focused rounding role — not full facility medical coverage
• Aeris APPs provide ongoing day-to-day clinical management

The covering physician will provide scheduled visits for post-acute skilled nursing residents recovering from hospitalization. Patients are typically receiving short-term rehabilitation or medical recovery services with the goal of returning to the community.

This is a focused physician rounding assignment, allowing you to concentrate on clinical evaluations and recommendations while the Aeris APP team continues to manage the residents' ongoing care.

• Perform scheduled physician visits for post-acute residents.
• Review recent hospitalization, diagnoses, medications, and current medical status.
• Evaluate therapy progress and overall plan of care.
• Assess whether the current treatment plan remains appropriate.
• Provide clinical recommendations to the primary Aeris APP when changes or additional follow-up are indicated.
• Complete required physician visit documentation.

The covering physician will not be responsible for routine day-to-day facility management or entering orders.

• Medications
• Orders
• Acute clinical concerns
• Ongoing primary care
• Day-to-day resident management
