Primary Care Physician (FM/IM) – Atlanta, GA | $250K–$300K | Outpatient Only
Ascend Medical is seeking a Board-Certified Family Medicine or Internal Medicine Physician to join its growing outpatient primary care practice in Atlanta's vibrant Old Fourth Ward community.
This is an excellent opportunity for a physician who enjoys building long-term patient relationships while practicing in a collaborative, team-based environment focused on prevention, chronic disease management, and whole-person care.
Job Responsibilities
Outpatient-only practice
Full-time or part-time schedule available
Competitive compensation: $250,000–$300,000
Collaborative care model with APPs, behavioral health, nutrition, and care coordination teams
Strong focus on prevention, wellness, and population health
Opportunity to provide mentorship and clinical oversight to APPs

Responsibilities
Provide comprehensive outpatient primary care
Manage acute and chronic medical conditions
Perform wellness visits, preventive screenings, and routine follow-ups
Coordinate care with specialists and interdisciplinary team members
Support patient education and shared decision-making
Maintain accurate EMR documentation
Participate in quality improvement and value-based care initiatives
Qualifications
MD or DO
Board Certified in Family Medicine or Internal Medicine
Active Georgia medical license or ability to obtain
Active DEA registration
BLS certification
Strong outpatient primary care experience
Excellent communication and leadership skills
Preferred
Medicare enrollment or eligibility
2+ years of outpatient primary care experience
Experience with value-based or team-based care models
