Opportunity Overview
A patient-centered community health organization in Central Ohio is seeking a dedicated Family Medicine Physician to join an integrated care team.
This opportunity provides comprehensive primary care to patients across the lifespan, with an emphasis on preventive medicine, chronic disease management, acute care, and coordinated whole-person healthcare.
The physician will work alongside professionals across medical, dental, behavioral health, nursing, and care coordination disciplines, creating a collaborative environment focused on accessible, comprehensive, and patient-centered care.
Job Responsibilities
Deliver preventive, acute, and chronic disease management services.
Conduct patient evaluations, physical examinations, and diagnostic assessments.
Order, interpret, and appropriately act on laboratory and diagnostic testing.
Develop and manage evidence-based treatment plans.
Evaluate and manage same-day and acute outpatient presentations.
Coordinate referrals and collaborate with specialists when appropriate.
Maintain accurate and timely documentation in the electronic health record.
Collaborate with dental, behavioral health, nursing, and care coordination teams.
Participate in quality improvement and population health initiatives.
Provide patient education related to prevention, wellness, and disease management.
Participate in community health initiatives consistent with an integrated medical home model.
Support culturally responsive and trauma-informed care practices.
Qualifications
Completion of an accredited Family Medicine residency.
Board Certified or Board Eligible in Family Medicine.
Family Medicine Obstetrics certification/experience preferred or required, depending on final client criteria.
Active or eligible for an unrestricted Ohio medical license.
Active or eligible for DEA registration.
Strong clinical skills in full-scope Family Medicine.
Ability to manage acute and same-day outpatient visits.
Experience or strong interest in community health and underserved populations.
Strong communication, interpersonal, and teamwork skills.
Proficiency with electronic health records.
Commitment to patient-centered, culturally responsive care.
Working Hours
Full-time outpatient Family Medicine position.
