Job description
We are seeing for a Primary Care Physician (MD/DO) to provide comprehensive, patient-centered primary and preventive care. The ideal candidate will be committed to improving patient outcomes, managing acute and chronic conditions, and working collaboratively with an integrated healthcare team in an FQHC setting.
Job Responsibilities
Provide comprehensive primary and preventive care to patients
Diagnose and treat acute and chronic medical conditions
Develop, implement, and manage individualized treatment plans
Order, review, and interpret laboratory, imaging, and other diagnostic testing
Prescribe and manage medications appropriately
Coordinate referrals and specialty care as needed
Collaborate with behavioral health, pharmacy, and other multidisciplinary care teams
Educate patients on treatment plans, disease prevention, and healthy lifestyle practices
Maintain accurate, complete, and timely medical documentation
Participate in quality improvement and FQHC performance initiatives
Promote high-quality, patient-centered care and continuity of care
Qualifications
MD or DO degree from an accredited medical school
Current, unrestricted Ohio medical license or eligibility for Ohio licensure
Board certified or board eligible in the applicable specialty
Current DEA registration or ability to obtain one
Strong clinical, communication, and interpersonal skills
Commitment to providing high-quality, compassionate patient care
Ability to work effectively as part of a multidisciplinary healthcare team
