Family Medicine Physician needed ASAP

Practice Setting & Schedule

• Practice Environment: Rural outpatient primary care within an established, Patient-Centered Medical Home (PCMH) and Federally Qualified Health Center (FQHC) network operating since 1974.
• Work Week: Full-time, standard Monday through Friday schedule with no weekend clinic hours.
• Holiday Observance: Clinic is closed on all major holidays.
• Patient Flow: Outpatient volume of 20+ patients per day across all age groups (pediatric through geriatric).
• Clinical Documentation: Uses eClinicalWorks (eCW) for outpatient electronic medical records.

Assignment Scope & Contract Logistics

• Initial Duration: 3-month contract assignment with strong potential to extend.
• Permanent Transition: Prefer candidates that would consider a transition after 4 months of Locums
• Travel & Accommodations: Full travel coordination and lodging accommodations provided throughout the assignment.
• Credentialing Window: Approximately 30 to 60 days for complete verification; start date begins immediately upon full clearance.
• Interview Format: Streamlined panel interview conducted via Zoom with executive C-suite leadership.

Multidisciplinary Team & Network Support

• Extensive Regional Reach: Backed by a regional network of 18 family and internal medicine offices, dedicated pediatrics, OB/GYN clinics, and school-based health centers.
• Comprehensive Care Coordination: In-house case managers, RN ACO care managers, and community health workers manage care plans, follow-ups, and patient tracking.
• Integrated Ancillary Care: Seamless internal referrals to on-site behavioral health (psychiatrists and LCSWs), three dental practices, and four in-house pharmacies with a 340B prescription drug program.
• Specialized Community Outreach: Active field support teams dedicated to chronic disease management, diabetes education, infectious disease, cancer screening, and migrant/seasonal farmworker programs.
• Collaborative Medical Staff: Practice alongside a broad panel of family physicians, internists, pediatricians, OB/GYNs, advanced practice providers, clinical pharmacists, and dentists.

Candidate Requirements & Qualifications

• Specialty Training: Board Certification in Family Medicine required.
• Licensure & Credentials: Active, unrestricted DEA registration and a clean professional background.
• Scope of Practice: Demonstrated clinical comfort treating the full spectrum of all ages in a high-need, rural outpatient setting.
• Core Competencies: Proven commitment to sound clinical judgment, strong teamwork and communication, accountability, and mission-driven patient care.
