Job Responsibilities
• Conduct patient evaluations, diagnoses, and individualized treatment planning

Qualifications
• Board Certified or Board Eligible in Family Medicine
• Active Oregon medical license or eligibility to obtain one
• DEA registration
• Current CPR certification
• Experience in outpatient practice or community health preferred
