Excellent full-time Primary Care Physician opportunity in San Jose, California! Join a mission-driven FQHC serving diverse and multicultural communities throughout Santa Clara County. This role offers the opportunity to provide comprehensive, patient-centered outpatient care while working alongside a collaborative multidisciplinary team integrating primary care, behavioral health, and community wellness services.

Job Responsibilities
• Conduct examinations, interpret diagnostic studies, diagnose conditions, and develop individualized treatment plans.
• Manage chronic diseases through treatment, education, prevention, and self-care guidance.
• Coordinate specialty referrals, inpatient care, and collaborate with behavioral health and wellness teams.
• Participate in quality improvement, peer reviews, clinical audits, and community health initiatives.
Qualifications
• Board Certified in Family Medicine, Internal Medicine, or related specialty.
• Current, unrestricted California medical license.
• Active, unrestricted DEA registration.
• Minimum 2 years of primary care clinical experience.
• Current BLS certification.
• Ability to manage a full-time patient panel of approximately 1,500 patients.
• Strong commitment to serving underserved and multicultural communities.
• Excellent communication and collaborative skills.
• English proficiency required; additional language proficiency is a plus.

• Rotational after-hours physician call
