We are seeking a physician for a flexible locum tenens Home Health role in Connecticut (Windham and surrounding counties) with Signify Health. This role involves conducting pre-scheduled in-home health evaluations. Please see below for more information.
- Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members
- Visits include medication and medical history review plus a physical evaluation in the member’s home
- No prescribing, lab ordering, or altering current treatment regimens — a pre-populated, patient-specific form is provided for each evaluation
- Average 6-9 visits daily; flexible schedule
- mySignify EMR
- New graduates accepted; multiple positions available across Connecticut counties
Job Responsibilities
• Perform medication and medical history reviews and physical evaluations in the home
• Complete a pre-populated, patient-specific form for each evaluation
• Average 6-9 visits per day
• No prescribing, lab ordering, or altering treatment regimens
Qualifications
• Active Connecticut license in hand required
• Master’s degree required
• DEA not required (no prescribing or diagnosing)
• New graduates accepted
• Credentialing timeframe: approximately 6-8 weeks
