Family Medicine Opening Logansport & Peru, IN
Full Time | Family Medicine
Seeking a BC/BE Family Medicine Physician to provide outpatient primary care at two clinic locations in Logansport and Peru, Indiana.
Position Highlights:
100% outpatient practice
Split schedule: 2 days/week in Logansport and 2 days/week in Peru
Flexible weekday schedule
No weekends, evenings, or major holiday coverage
Team-based, collaborative care model
Onsite Integrated Behavioral Health

Qualifications:
MD or DO
Completion of an accredited Family Medicine residency
Board Certified or Board Eligible
Licensed or eligible for Indiana medical licensure
Active DEA license or ability to obtain prior to start
FQHC, Community Health Center, or Patient-Centered Medical Home experience preferred
