Family Medicine Physician

We are seeking a Family Medicine Physician to join a mission-driven FQHC practice on Long Island. This role offers a well-structured schedule, strong clinical support, and the opportunity to serve a diverse patient population within an established multidisciplinary care model.

Role Overview

• Schedule: Monday–Friday, 8:00am–5:00pm. One evening shift per week (11:00am–8:00pm; includes 8 hours of patient care with 1 unpaid meal break). One Saturday per month; provider receives a weekday off when working Saturday.
• Patient Volume: Approximately 20–25 patients per day.
• Clinical Scope: Adult primary care. Pediatric care supported by on-site Pediatrics Departments. Women’s health services available across all sites (including GYN and prenatal care).
• Call: Telephone-only call, handled remotely from home. 2–3 weeks per year (~once every six months). Triage nurse screens all calls; physician contacted only for urgent issues. Schedule distributed 6 months in advance.
• Support Model: Integrated FQHC environment with strong interdisciplinary collaboration.

Experience & Hiring Preferences

• Riverhead: Experience preferred. Will consider a new graduate who is a Chief Resident.

Requirements

• MD or DO, Board Certified / Board Eligible in Family Medicine.
• Active or eligible New York State medical license.
• Comfort working in an FQHC or community health setting preferred.
• Commitment to collaborative, team-based care.
• For Wyandanch: comfort with teaching and precepting responsibilities.
