Job Responsibilities
• Develop long-term relationships with patients and families through proactive, preventive healthcare
• Diagnose and manage chronic conditions including:
Diabetes
Hypertension
COPD
Congestive Heart Failure (CHF)
Chronic Kidney Disease (CKD)
• Perform preventive care evaluations and wellness-focused visits
• Coordinate care with multidisciplinary teams including advanced practice providers, pharmacists, and care coordinators
• Utilize EMR systems, clinical data, and quality tools to support patient outcomes
• Participate in quality improvement initiatives focused on patient experience and healthcare outcomes
• Provide evidence-based medical care while maintaining a patient-centered approach
Qualifications
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified or Board Eligible in:
Family Medicine
Internal Medicine
Geriatrics
• Active Florida medical license or ability to obtain
• Experience providing outpatient primary care
• Experience managing complex chronic conditions
Preferred:
• 3+ years outpatient primary care experience
• Experience caring for senior or Medicare patients
• Experience with value-based care models
• Strong communication and relationship-building skills
Working Hours
No nights
No weekends

Full-time outpatient schedule.
