Job description
Join a nation-wide primary care organization focused on the geriatric population. We are seeking advanced practice providers for our South Carolina locations.

Work Schedule: Monday-Friday, 8am-5pm (no nights or weekends)
- Patient Load: 12-16 patients per day once fully ramped up
- Patient Demographics: Primarily Medicare (over age 65) with some Medicaid (up to 25% under age 65)
- Collaborative Care Team: Includes 1 MD, APP, Scribe, MA, and shared RN/Social Work support
- Specialty Support: Partnership with an outside telemedicine company for quick specialist consultations
- Technology: EPIC EMR managed by scribes, advanced algorithm for patient visit predictions
- Call Schedule: Rotated amongst the market, on average 1 week every 3 months, with call center support for limited and remote calls
Position Details
This is a full-time permanent position with a 4-6 week orientation/training period. Benefits include stock options, extensive PTO, large CME stipend, malpractice coverage, and 401k with match available from day one.
Requirements
- Board Certification Required
- SC State License Required
- Training in family medicine, internal medicine, or geriatrics
- Previous primary care or skilled nursing facility/nursing home experience preferred, but not required

Our organization provides mobile vans to transport patients to and from clinics, ensuring access to care for geriatric patients. The goal is to keep Medicare patients out of the hospital through a value-based care model. Team-based approach with quality bonuses given to all staff at sites.
