About the Practice

You'll provide comprehensive outpatient primary care for senior adults in a modern, relationship-driven environment focused on preventive medicine and chronic disease management.

Unlike traditional fee-for-service practices, this organization emphasizes meaningful physician-patient relationships, proactive healthcare, and improved long-term outcomes.

You'll have the autonomy to practice medicine while receiving exceptional clinical, operational, and technological support.

What You'll Do

• Deliver comprehensive primary care services to Medicare Advantage and senior patients

• Build long-term relationships with patients and their families

• Diagnose and manage acute and chronic medical conditions

• Develop individualized treatment plans

• Focus heavily on preventive care and wellness initiatives

• Manage chronic diseases including:

  • Diabetes

  • Hypertension

  • COPD

  • Congestive Heart Failure

  • Chronic Kidney Disease

• Coordinate care with specialists when appropriate

• Utilize EMR tools and quality dashboards to improve patient outcomes

• Collaborate with advanced practice providers, pharmacists, care coordinators, and support staff

• Participate in value-based care initiatives focused on quality metrics and patient satisfaction

Qualifications

Required

• MD or DO

• Board Certified or Board Eligible in:

  • Family Medicine

  • Internal Medicine

  • Geriatrics

• Active Florida Medical License (or ability to obtain)

• DEA Registration

• BLS/ACLS as required

Preferred

• Three or more years of outpatient primary care experience

• Experience treating Medicare Advantage patients

• Value-Based Care experience

• Strong chronic disease management skills

• Excellent communication and relationship-building abilities
