Job Responsibilities
• Collaborate with multidisciplinary teams during daily clinical huddles and care planning
• Coordinate specialty referrals, hospital transitions, SNF coordination, DME, and home health services
• Maintain timely and accurate EMR documentation with coding support assistance
• Participate in quality improvement initiatives and patient outcome optimization
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible Alabama medical license
• Active DEA license required
• Medicare and Medicaid provider eligibility preferred
Working Hours
• Dedicated administrative and charting support provided
Skills
• Value-based care experience preferred

• EMR proficiency
• Knowledge of HEDIS/STARS, CAHPS, or Medicare Advantage models preferred
