Job Responsibilities
• Coordinate specialty referrals, hospital follow-ups, SNF coordination, home health, and durable medical equipment needs
• Participate in daily team huddles and collaborative care planning
• Maintain accurate and timely EMR documentation
• Review quality outcomes and patient care initiatives with leadership
• Deliver patient-centered care focused on preventive medicine and chronic disease management
Qualifications
• Active and unrestricted medical license or willingness to obtain licensure in assigned state
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine preferred
• Active DEA license required
• Strong communication and interpersonal skills
• Comfortable working within a team-based, value-care environment
• EMR proficiency required
