Family Medicine Physician – Outpatient Primary Care

Location: Sonoma County / North Bay, California
Position Type: Full-Time | Employed
Schedule: 4–5 days per week
Setting: Outpatient Clinic
EMR: Epic

Position Overview

A well-established, physician-led medical group in the Sonoma County area of Northern California is seeking a Board-Certified or Board-Eligible Family Medicine Physician to join a busy outpatient primary care practice.

This is an excellent opportunity to step into an established patient panel with strong community demand, robust referral support, and an experienced clinical team. The practice offers a collaborative environment focused on continuity of care, quality outcomes, and physician work-life balance.

Job Responsibilities

Provide full-spectrum outpatient family medicine care to patients of all ages
Manage acute visits, chronic disease follow-up, and preventive wellness care
Diagnose and treat common primary care conditions
Perform annual physicals, Medicare wellness visits, and health screenings
Develop evidence-based care plans and treatment strategies
Manage chronic conditions including:
Hypertension
Diabetes
Hyperlipidemia
Thyroid disease
Asthma/COPD
Coordinate specialist referrals, imaging, and follow-up care
Document thoroughly and efficiently in Epic
Participate in patient messaging, prescription refills, and results management
Collaborate with APPs, MAs, nursing staff, and specialty partners

Qualifications

MD or DO
Board Certified / Board Eligible in Family Medicine
Active California license or ability to obtain
DEA required
New graduates welcome
Strong communication and patient engagement skills
Interest in long-term community-based primary care

Working Hours

Monday–Friday clinic schedule
Flexible 4-day workweek option available
No inpatient responsibilities
Shared light phone call only
Supported by RN triage/advice line
