Job Responsibilities
• Manage chronic and acute medical conditions
• Perform preventive care, screenings, and immunizations
• Collaborate with APPs and care teams for coordinated care
• Participate in light call rotation with RN triage support
• Optionally engage in teaching and mentorship activities
Qualifications
• Active New York license or willingness to obtain
• Minimum 1 year of primary care experience required
• Clean malpractice and licensing history
Working Hours
• No routine weekends
• Minimal call: phone coverage every ~70 days + limited telehealth weekends
Skills
• Preventive care and chronic disease management expertise
• Excellent communication and teamwork
