• Must have 3+ years of leadership experience overseeing physicians • Leadership position with a 50% leadership / 50% clinical split • Work schedule: Monday–Friday, 7:30a–5p • Full-time position • Value-based care model • See patients on a monthly basis • Small patient panels, typically 350–450 patients per physician • On-call responsibilities limited to your own patient panel • In most situations, physicians are not required to supervise APPs • Clinical population consists primarily of Medicare Advantage patients age 65 and older • Average patient age is approximately 71–72, often with multiple chronic conditions • Focus on geriatric primary care and complex chronic disease management • The practice does not typically see children or younger adults • EMR: proprietary platform • No 2026 or 2027 residents • No visa sponsorship available

About the Client: Physicians who work for this organization have a non-compete in their contract; however, it is limited to capitated practices only. A capitated medical practice receives a fixed, predetermined payment for each patient assigned to them over a specific period, typically on a per-member, per-month basis, regardless of how many services the patient uses.
The care model is built around value-based, preventive medicine with a focus on proactive care, chronic disease management, and strong physician-patient relationships. The organization partners with Medicare Advantage health plans to deliver coordinated care designed to improve outcomes and reduce hospitalizations. Physicians work within a team-based model that includes nurses, care coordinators, behavioral health specialists, and social workers, allowing providers to focus on prevention and long-term health management rather than high-volume episodic care.
