Job Responsibilities
• Evaluate, diagnose, and treat patients across all age groups
• Develop and implement individualized patient care and treatment plans
• Maintain accurate documentation in the electronic medical record system
• Collaborate with physicians, advanced practice providers, and healthcare staff to deliver quality care
• Participate in after-hours on-call rotation with Family Medicine colleagues
• Contribute to care improvement initiatives and patient care strategies
Qualifications
• Board Certified or Board Eligible in Family Medicine (must complete residency)
• Colorado medical license or willingness to obtain
• Clean malpractice history preferred
• DEA registration required prior to start
• NPDB Self-Query report (within 30 days) required at time of submission
Working Hours
• Approximately 36 patient-facing hours per week
• Day shifts
• Participation in after-hours on-call rotation with team members
Skills
• Chronic disease management and patient care coordination
• Clinical diagnosis and treatment planning
• EMR documentation and clinical workflow management
• Strong communication and patient relationship skills
• Ability to work effectively in a team-based healthcare environment
