Position Summary

The facility is seeking a Primary Care Physician / Family Medicine Physician to join its healthcare team in Bronx, NY . Both full-time and part-time opportunities are available.

Essential Position Responsibilities

Provide comprehensive outpatient primary care services for patients of all ages.

Perform routine physical examinations, annual wellness visits, and preventive screenings.

Diagnose and manage acute and chronic medical conditions such as diabetes, hypertension, asthma, and cardiovascular disease.

Order, interpret, and follow up on diagnostic tests and imaging.

Develop and implement individualized treatment plans.

Provide patient education on preventive health, lifestyle modification, and disease management.

Coordinate referrals to specialists and collaborate with interdisciplinary team members.

Maintain accurate and timely documentation within the electronic health record (EHR).

Participate in quality improvement initiatives and team meetings.

Qualifications

Education:

MD or DO degree from an accredited medical school.

Board Certification:

Board Certified or Board Eligible in Family Medicine or Internal Medicine.

Licensure:

Active or eligible medical license in New York.

Experience:

Prior outpatient primary care experience preferred; new graduates are welcome to apply.
