Description:
A mission-driven Federally Qualified Health Center (FQHC) in Connecticut is seeking a Primary Care Physician (MD or DO) to join its outpatient clinical team. This role provides the opportunity to deliver comprehensive, evidence-based primary care to patients across the lifespan while working within a collaborative, multidisciplinary care model.
Reporting to the Medical Director, this physician will provide direct patient care, support coordinated and integrated services, and play a meaningful role in training future family physicians to serve diverse, multicultural, and underserved populations. Precepting opportunities are also available for those who are interested.
Job Responsibilities
- Provide comprehensive outpatient primary care services for patients of all ages, from pediatrics through geriatrics
- Conduct patient evaluations, including medical histories, physical exams, diagnostic testing, and follow-up care
- Diagnose and manage acute, chronic, and complex medical conditions
- Prescribe medications and treatment plans in accordance with Connecticut state regulations and clinical guidelines
- Educate patients on disease prevention, health maintenance, diet, activity, and lifestyle modifications
- Refer patients to specialists or higher levels of care as clinically appropriate
- Document patient encounters thoroughly and accurately in the medical record
- Participate in quality improvement initiatives, utilization reviews, and quality assurance activities
- Collaborate with clinical and operational teams to support efficient, coordinated patient care
- Participate in on-call rotation as required
- Support Accountable Care Organization (ACO) activities and population health initiatives
- Contribute to the training and mentorship of future family physicians
Qualifications
- MD or DO from an accredited medical school
- Completion of an accredited residency program in an appropriate specialty
- Board Certified or Board Eligible preferred
- Active, unrestricted Connecticut medical license
- Valid federal and state controlled substance licenses
- Experience in an outpatient primary care or community health setting preferred
- Strong interest in serving low-income, multicultural, and underserved patient populations
- Ability to work effectively within a multidisciplinary team environment
- Working knowledge of ICD-10, CPT, and HCPCS coding preferred
