About the Opportunity:

• Join a nationally recognized home-based primary care organization focused on delivering personalized care directly in patients' homes
• Practice medicine in a setting that provides greater visibility into patients' health, medication adherence, nutrition, and living conditions
• Improve patient outcomes while reducing unnecessary emergency room visits and hospitalizations
• Combination of clinical practice and physician leadership responsibilities
• Lead and mentor a team of approximately 4-5 Nurse Practitioners
• Reduced physician panel allows for meaningful patient interactions and leadership engagement
• Strong emphasis on collaborative, team-based care
• Weekly clinical huddles and dedicated collaboration time
• Highly supportive operational infrastructure and centralized call center
• Monday-Friday schedule
• No weekend clinic responsibilities
• Call rotation approximately one week every two months

Compensation:

• Base salary of $265,000 - $300,000
• Leadership stipend of $5,000 annually per Nurse Practitioner supervised
• Additional $20,000 leadership incentive for overseeing four Nurse Practitioners
• Annual performance bonus of $20,000 - $60,000 based on productivity, quality outcomes, and reduced ER utilization
• Total earning potential of $350,000+
• $50,000 retention bonus paid over two years

Candidate Parameters:

• Board Eligible or Board Certified in Family Medicine or Internal Medicine
• Eligible for Virginia medical licensure
• Geriatrics experience or interest preferred
