Base Salary: $240,000 – $260,000 + Production Incentives

📋 Requirements:
-
Board Certified or Truly Board Eligible (within 5 years of residency) in Family Medicine
-
Indiana medical license (or willing to obtain)
-
BLS & ACLS (or willing to obtain)
-
DEA (or willing to obtain)
-
COVID-19 vaccination required
-
Must have experience seeing patients of all ages (peds–geriatrics)

EPIC EMR experience highly preferred
