Consider this new opportunity to serve as an Outpatient Medical Director for a Critical Access Hospital in Northern Arizona. Based on referral volumes and community need, the incoming provider should be full quickly.

- Serve as Supervising Provider for the Patient Centered Medical Home program
- 50% clinical and 50% Administrative
- Provide direct supervision of medical staff members assigned to the outpatient departments
- Reports to Chief Medical Officer
- At least 3 years experience as a Medical Supervisor in an outpatient clinic setting
- Strong familiarity with the Patient Centered Medical Home model of care and understanding in basic concepts in Population Health Management.
