The primary care physician will provide health assessments, diagnoses of acute and chronic illnesses, as well as formulate treatment plans.
Qualifications: Doctor of Medicine Degree or its foreign equivalent. Physician License from the State of Indiana or at the time of hire. Current DEA licensure and RX authority from the state of Indiana or at the time of hire. Board Certified or Board Eligible in related field.
