*Location: Polacca, AZ; NE AZ, Indian Health Services Facility; Gorgeous area and great weather!
*Location: Polacca, AZ; NE AZ, Indian Health Services Facility; Gorgeous area and great weather!
*Setting: Walk-in Clinic
*Shifts/Duration: 8 hour days, 2 wks on / 2 wks off, Ongoing during 2023; Monday-Friday work week
*Volume: average 12 patients per day; adults and peds
*Requirements:
- Any active, unrestricted State License
- Clean DEA/Malpractice/NPDB (no current suits, unresolved cases, or large payouts)
- FM Board Certification, with 2 years experience; Recent Outpatient experience preferred
- No lapse in practice greater than 6 months leading up to assignment
- Security clearance screening through Inspector General
- The clearance/credentialing process is generally about 3 months because of the Security Clearance
- 3-6 Month Minimum Commitment
*Rate: $140/hour; travel, housing and malpractice insurance included
