- 3 locations and over 30 Providers and growing
- Provide services in primary care, dental and behavioral health
- 32 patient contact hours, including one telehealth shift
- Patient-Centered Medical Home
- The average daily patient census is 14-20
- On-call is just 4 weeks per year
