Schedule = 18 PPD, the minimum is 18 PPD
Monday-Friday 8-5
A mix of peds/adults
Needs experience with Chronic Pain Management
A decision is typically made within 7-14 days
We will also accept DO
APCs with 3 years of experience in a similar setting (see other AR)
BE will be considered if indeed BE (5 years or less since residency)
Active DEA needed before the start
Credentialing takes 45-90 days
The following immunizations are required for credentialing (before the start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Covid-19 Vaccine Series and 1 Booster
