Our integrated healthcare model includes primary medical care, behavioral health, dental services, OB/GYN and family planning, pharmacy services, laboratory and imaging support, case management, transportation assistance, specialty referrals, social work services, and the WIC program.
We are seeking a compassionate and dedicated Primary Care Physician to join our team and help advance our mission of improving the health and quality of life of the communities we serve.
The Primary Care Physician provides comprehensive medical care to patients across the lifespan in a patient-centered primary care setting. This role is responsible for evaluating, diagnosing, treating, and managing acute and chronic health conditions while promoting preventive care and overall wellness.
The Physician will collaborate with an interdisciplinary care team, including behavioral health providers, dentists, pharmacists, social workers, and case managers, to deliver coordinated and culturally sensitive healthcare.

Job Responsibilities
Patient Care:
Provide comprehensive primary care services for pediatric, adult, and/or family medicine patients.
Perform patient assessments, physical examinations, diagnoses, and treatment planning.
Manage acute and chronic medical conditions using evidence-based practices.
Prescribe medications and provide appropriate medical therapies.
Order, review, and interpret diagnostic tests and laboratory results.
Provide preventive healthcare services, health education, and wellness counseling.
Coordinate specialty referrals and follow-up care when needed.
Care Coordination & Collaboration:
Work collaboratively with an interdisciplinary healthcare team, including:
Behavioral health professionals
Dental providers
Pharmacists
Social workers
Case managers
Other clinical support staff
Support integrated care approaches that address medical, behavioral, and social health needs.
Participate in team-based discussions to improve patient outcomes.
Documentation & Quality Improvement:
Maintain accurate, timely, and complete documentation within the electronic health record (EHR).
Ensure compliance with clinical, regulatory, and quality standards.
Participate in quality improvement initiatives and organizational performance programs.
Support preventive care initiatives and community health improvement efforts.
Professional Practice:
Provide respectful, culturally sensitive, and patient-centered care.
Maintain ethical and professional standards of medical practice.
Contribute to a safe, collaborative, and supportive clinical environment.
Support Client’s mission, values, and commitment to community health.
Qualifications
Required:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
Completion of an accredited residency program in:
Family Medicine, or
Internal Medicine
Current unrestricted medical license, or eligibility for licensure, in Alabama.
Board certification or board eligibility in Family Medicine or Internal Medicine.
Strong clinical knowledge and experience managing primary care conditions.
Excellent communication and interpersonal skills.
Ability to work effectively within an interdisciplinary healthcare environment.
Commitment to compassionate, patient-focused care.
Preferred:
Experience working in a Federally Qualified Health Center (FQHC), community health center, or similar integrated care environment.
Experience serving diverse and underserved patient populations.
Familiarity with electronic health record systems and quality improvement initiatives.
