We are seeking a dedicated and compassionate Family Physician to join our integrated care team in Marion, Ohio. This role provides comprehensive primary care services across all age groups, with a strong emphasis on preventive medicine, chronic disease management, and coordinated care delivery within a team-based model.

Job Responsibilities
Conduct patient evaluations, physical exams, and diagnostic assessments
Order, interpret, and act on laboratory and diagnostic testing
Develop and manage evidence-based treatment plans
Manage same-day visits and acute outpatient needs, including urgent care presentations
Coordinate referrals and collaborate with specialty providers as needed
Document patient care accurately in the electronic health record (EHR)
Work closely with dental, behavioral health, nursing, and care coordination teams to support integrated care delivery
Participate in quality improvement initiatives and population health efforts
Provide patient education focused on prevention, wellness, and disease management
Engage in community health initiatives aligned with the medical home model
Qualifications
Completion of an accredited Family Medicine residency program
Board certified or board eligible in Family Medicine
Board Certification in Family Medicine Obstetrics (preferred/required per posting)
Active or eligible for an unrestricted Ohio medical license and DEA registration
Skills
Experience or strong interest in community health and underserved populations
Ability to manage acute and same-day outpatient visits
Strong interpersonal, communication, and teamwork skills
Proficiency with electronic health records and data-driven clinical care
Commitment to culturally responsive, trauma-informed, and patient-centered care
