We are seeking a dedicated Family Medicine or Internal Medicine Physician to provide comprehensive primary care services at an established intermediate care facility in Dixon, Illinois. Job Responsibilities
Complete annual physical examinations and required reviews
Perform quarterly medication management and follow-up assessments as needed
Provide ongoing primary care services to residents with developmental disabilities
Participate in structured rounds between residential homes on campus
Attend occasional meetings with Medical Directors and care teams
Ensure timely documentation and compliance with facility standards
