Take Ownership of Patient Care. Help Lead the Clinical Team.

An integrated primary care practice in Atlanta’s Old Fourth Ward is seeking a Board-Certified Family Medicine or Internal Medicine Physician with a strong interest in caring for older adults and patients with complex chronic conditions.

This opportunity combines hands-on outpatient medicine with meaningful clinical leadership. We are seeking a physician who confidently evaluates patients, establishes clear treatment plans, follows through on outstanding concerns, and helps the broader care team navigate clinical decisions.

You will work alongside advanced practice providers, nurses, care coordinators, behavioral health professionals, and nutrition specialists to deliver coordinated, whole-person care.

The strongest fit is a physician who is approachable and collaborative while remaining decisive, accountable, and comfortable taking the clinical lead.

Your Clinical Responsibilities

  • Provide comprehensive outpatient primary care, with an emphasis on older adults, prevention, and chronic disease management.

  • Evaluate acute concerns, establish appropriate differential diagnoses, and determine treatment, testing, referral, or escalation needs.

  • Manage patients with multiple chronic conditions, balancing evidence-based treatment with functional status, patient preferences, and realistic health goals.

  • Review medication regimens for effectiveness, adherence, interactions, and opportunities to simplify treatment when appropriate.

  • Perform preventive visits, Medicare annual wellness visits, and appropriate screenings.

  • Recognize changes in cognition, mobility, function, and social support that may affect an older adult’s health and safety.

  • Involve family members and caregivers when appropriate and consistent with patient consent.

  • Coordinate specialist care and follow-up after emergency department visits or hospitalizations.

  • Maintain timely documentation and ensure results, referrals, and outstanding clinical concerns receive appropriate follow-through.

Clinical Leadership & APP Collaboration

  • Serve as a dependable clinical resource for APPs and other team members.

  • Provide clear, timely guidance on complex cases, diagnostic questions, treatment decisions, and escalation needs.

  • Participate in case review, mentorship, and collaborative oversight as appropriate to the agreed role.

  • Help establish consistent clinical standards and practical care protocols.

  • Address gaps in care proactively and recommend improvements to workflows and care coordination.

  • Support a team culture built on communication, accountability, patient safety, and mutual respect.

Additional compensation is available for agreed APP supervision responsibilities. The number of APPs and scope of responsibilities will be discussed with the physician during the interview process.

What We Are Looking For

  • MD or DO degree.

  • Board certification in Family Medicine or Internal Medicine.

  • Active, unrestricted Georgia medical license or ability to obtain licensure before starting.

  • Current DEA registration.

  • Current BLS certification or ability to obtain before starting.

  • Strong outpatient diagnostic skills and sound independent clinical judgment.

  • Experience or demonstrated competence caring for older adults and patients with multiple chronic conditions.

  • Ability to communicate a clear clinical assessment, explain the plan, and guide next steps.

  • Willingness to take ownership of patient outcomes and provide practical support to APP colleagues.

  • Comfort using an EMR and maintaining organized, timely follow-up.

  • Local residence within commuting distance of the Atlanta practice.

Preferred Background

  • Two or more years of outpatient primary care experience after residency.

  • Experience with geriatric populations, Medicare patients, or medically complex adults.

  • Prior APP collaboration, mentorship, or supervision.

  • Experience in integrated care, population health, or value-based care.

  • Interest in incorporating behavioral health, nutrition, and preventive strategies into primary care.

Geriatric fellowship training is welcome but is not required.

Practice Environment & Benefits

  • Full-time annual compensation of $250,000–$320,000, with final terms based on experience and agreed responsibilities.

  • Full-time and part-time opportunities; part-time compensation and benefits discussed individually.

  • Approximately 15–20 patient visits per day.

  • No weekend call or after-hours responsibilities.

  • Athena EMR.

  • Interdisciplinary clinical support.

  • Benefits package, including 401(k), vacation, and paid time off, subject to plan terms and eligibility.

  • Opportunity to contribute to clinical protocols, care delivery, and practice improvement.
