Ongoing coverage with locums-to-perm potential
Interdisciplinary team support
Assignment Details
November 2026 start; ongoing
Monday–Friday, 8:00 a.m.–5:00 p.m.; 3–4 days per week considered
Minimum two weeks per month; full-time coverage preferred
No call
Approximately 4–6 patients per shift
Outpatient clinic and community visits to assisted living, skilled nursing, and private residences
Eleven-person interdisciplinary team; a nurse usually accompanies community visits

Clinical Details
Care for adults age 55 and older in a PACE program
Geriatric experience required
Coordinate care with the interdisciplinary team across clinic and community settings
Requirements
Board certified or board eligible in Internal Medicine or Family Medicine
Active Oregon license and DEA required
Current BLS certification
