An innovative, integrated primary care practice in Old Fourth Ward, Atlanta is seeking a Board-Certified Family Medicine or Internal Medicine Physician (MD/DO) to join its growing team.
This is an outpatient primary care opportunity for a physician who values whole-person care, preventive medicine, long-term patient relationships, and interdisciplinary collaboration.

Job Responsibilities
Evaluate and manage acute and chronic medical conditions
Perform preventive visits, annual wellness exams, screenings, and routine examinations
Develop individualized treatment plans
Order and interpret diagnostic testing
Manage medications and monitor treatment response
Educate patients and promote healthy lifestyle choices
Coordinate care with specialists, hospitals, and community providers
Collaborate with behavioral health, nutrition, nursing, APPs, and care coordination teams
Provide clinical consultation, mentorship, and collaborative oversight to APPs
Participate in quality improvement, population health, and patient-safety initiatives
Help develop and improve clinical protocols and care processes
Qualifications
Board Certification in Family Medicine or Internal Medicine
Active, unrestricted Georgia medical license or ability to obtain one
Current DEA registration
Current BLS certification or ability to obtain
Strong clinical judgment and communication skills
Commitment to evidence-based, patient-centered outpatient medicine
Preferred Qualifications
2+ years of outpatient primary care experience after residency
Medicare enrollment or eligibility
Experience in interdisciplinary or value-based care
Experience mentoring or collaborating with APPs
Interest in preventive medicine, population health, nutrition, and lifestyle medicine
Practice Environment
The physician will work in an integrated outpatient primary care setting alongside behavioral health providers, nutrition professionals, APPs, nurses, and care coordinators. The practice emphasizes continuity of care, prevention, care coordination, and meaningful long-term patient relationships.
