Primary Care Physician – Family/Internal Medicine | Geriatric Experience | $250K–$320K |Atlanta, GA HZ

Key Responsibilities:
Comprehensive Outpatient Care:
Evaluate, diagnose, and manage acute and chronic medical conditions using evidence-based clinical practices.
Provide primary care within your specialty, training, and clinical scope.
Conduct preventive health visits, annual wellness visits, routine examinations, and appropriate screenings.
Develop individualized treatment plans based on medical history, clinical findings, patient preferences, and health goals.
Order and interpret diagnostic testing and coordinate additional evaluation when appropriate.
Manage medications and monitor treatment effectiveness, tolerability, and adherence.
Educate patients regarding their conditions, treatment options, and opportunities to improve their health.
Build lasting patient relationships through clear communication, shared decision-making, and consistent follow-up.

Integrated & Preventive Medicine:
Incorporate behavioral health, nutrition, lifestyle, and preventive strategies into patient care when appropriate.
Identify social and access-related factors that may affect a patient's ability to follow a care plan.
Partner with interdisciplinary colleagues to address needs beyond the traditional medical visit.
Support early intervention, chronic disease prevention, and sustainable health improvements.
Coordinate care with specialists, hospitals, and community providers to maintain continuity across care settings.

Clinical Leadership & APP Collaboration:
Provide clinical oversight and collaborative supervision for APPs in accordance with applicable requirements and organizational policies.
Support APPs through consultation, case review, mentorship, and clinical guidance.
Assist the care team in navigating complex cases and determining appropriate next steps.
Promote consistent, evidence-based clinical care and collaborative decision-making.
Foster an environment where colleagues communicate openly, share knowledge, and seek clinical input when needed.

Quality & Practice Improvement:
Maintain timely, accurate, and complete electronic medical records.
Participate in quality improvement, population health, and patient-safety initiatives.
Contribute to the development and refinement of clinical protocols and care processes.
Support value-based care initiatives focused on prevention, coordination, and patient outcomes.
Maintain clinical, regulatory, and organizational standards.
Help create a welcoming, respectful, and patient-centered environment for patients, families, and colleagues.
