Job description
We are seeking a compassionate, board-certified or board-eligible physician to join a premier Program of All-Inclusive Care for the Elderly (PACE) in the San Francisco Bay Area.
This role is designed for a physician who values quality over volume. Operating under a capitated, fully integrated care model, you will be free from the constraints of fee-for-service billing and RVU-chasing. Instead, you will benefit from a low patient census, allowing you to spend meaningful 1:1 time with your patients. You will serve as the clinical leader of an Interdisciplinary Team (IDT), driving holistic, long-term health outcomes and helping seniors maintain their independence in the community.
Job Responsibilities
Conduct comprehensive initial, periodic, 6-month, and annual geriatric assessments.
Provide primary, acute, and chronic disease management for complex, frail seniors.
Order diagnostic tests, manage pharmacotherapy, and coordinate specialist referrals based on patient goals.
Maintain complete, timely clinical documentation in eClinicalWorks.
Interdisciplinary Leadership & Care Coordination
Serve as clinical lead for the Interdisciplinary Team (IDT), updating members during routine team meetings.
Collaborate with NPs, PAs, social workers, and therapists to develop, implement, and sign care plans.
Facilitate family care conferences to align patient support networks with treatment goals.
Provide medical oversight for clinic operations, ethics committees, infection control, and QAPI programs.
Transitional & Longitudinal Care
Partner with hospitalists on inpatient care, manage ED utilization, and coordinate safe discharges.
Oversee assigned nursing home participants, partnering with facility staff to ensure care plan adherence.
Participate in an infrequent, phone-only after-hours call rotation.
Qualifications
Formal training and Board Certification (or eligibility) in Internal Medicine or Family Medicine.
1+ years of clinical experience specializing in geriatrics, with a demonstrated passion for serving frail and elderly populations.
Current Basic Life Support (BLS) certification.
Active DEA certificate (or willingness to obtain upon offer).
Preferred: Prior experience in a PACE environment.
