Client is a Federally Qualified Health Center (FQHC) dedicated to providing comprehensive, accessible healthcare services to individuals and families throughout Central New Mexico.
With six health centers and one school-based health center, Client serves more than 40,000 patients annually through an integrated care model that combines primary medical care, dental services, and behavioral health support to improve overall health outcomes.

Client is seeking a compassionate and dedicated Family Physician (MD/DO) to provide comprehensive primary and preventive care to patients of all ages in a community health setting in Albuquerque, NM.
The Family Physician will deliver patient-centered medical care, diagnose and manage acute and chronic conditions, provide preventive health services, coordinate specialty referrals, and collaborate with an interdisciplinary healthcare team to support whole-person wellness.

Job Responsibilities
Primary Care Delivery:
Provide comprehensive family medicine services to patients across all age groups.
Conduct medical evaluations, physical examinations, and health assessments.
Diagnose and manage acute illnesses, chronic diseases, and preventive healthcare needs.
Provide routine screenings, wellness visits, and health maintenance services.
Deliver urgent care services and address same-day patient needs as appropriate.
Develop individualized treatment plans based on evidence-based medical practices.
Patient-Centered & Community Healthcare:
Provide compassionate, trauma-informed, and culturally sensitive care.
Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices.
Build strong relationships with patients and support improved health outcomes.
Address healthcare needs within underserved and diverse communities.
Participate in community health initiatives and wellness programs.
Care Coordination & Team Collaboration:
Collaborate with physicians, advanced practice providers, behavioral health professionals, nurses, and other healthcare team members.
Participate in integrated care delivery and population health initiatives.
Coordinate referrals to specialty providers and ensure continuity of care.
Contribute to quality improvement programs and clinical performance initiatives.
Documentation & Compliance:
Maintain accurate, timely, and complete documentation within the electronic health record (EHR).
Follow clinical guidelines, regulatory requirements, and organizational policies.
Support quality assurance and patient safety initiatives.
Participate in team meetings, training activities, and practice improvement efforts.
Required Qualifications:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
Completion of an accredited Family Medicine Residency Program.
Current, unrestricted medical license in the state of New Mexico or eligibility to obtain licensure.
Board Certification or Board Eligibility in Family Medicine.
Current DEA registration.
Current BLS and ACLS certification as required.
Strong clinical knowledge and experience in:
Family Medicine
Primary Care
Preventive Medicine
Acute and Chronic Disease Management
Proficiency with electronic health record (EHR) systems.
Excellent communication, documentation, and organizational skills.
Ability to work effectively in an interdisciplinary team environment.
Qualifications
Experience working in a Federally Qualified Health Center (FQHC), community health center, or safety-net healthcare environment.
Experience providing care to underserved or medically diverse populations.
Interest or experience in population health and value-based care.
Participation eligibility for programs such as the National Health Service Corps (NHSC) loan repayment program.
Experience with integrated behavioral health and team-based care models.
