Job Responsibilities
• Develop treatment plans focused on preventative care and chronic disease management
• Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams
• Help reduce avoidable hospitalizations and improve patient outcomes
• Complete EMR documentation and coding requirements
• Deliver compassionate, patient-centered care within post-acute and skilled nursing settings
• Participate in quality improvement initiatives and care coordination efforts
Qualifications
• Board Certified
• Strong clinical judgment and communication skills
• Experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, Urgent Care, Hospital Medicine, or Post-Acute Care preferred
• Passion for improving patient outcomes and quality of life
• EMR experience preferred
• Ability to work independently while collaborating within a multidisciplinary team

8:00 AM - 4:00 PM
