Position Overview

We are seeking a dedicated and compassionate Family Medicine Physician to join our growing primary care team in Stillwater, NY. The physician will provide comprehensive, patient-centered care to individuals and families across all age groups, focusing on preventive medicine, chronic disease management, and continuity of care. The ideal candidate is committed to building long-term patient relationships and delivering high-quality, evidence-based medical care.

Key Responsibilities

• Provide comprehensive outpatient primary care services for pediatric, adult, and geriatric patients.
• Conduct patient evaluations, physical examinations, and preventive health screenings.
• Diagnose and treat acute illnesses, injuries, and chronic medical conditions.
• Develop and implement individualized treatment plans based on patient needs and evidence-based practices.
• Order, interpret, and communicate laboratory, imaging, and diagnostic test results.
• Prescribe medications and monitor treatment effectiveness.
• Administer immunizations and preventive healthcare services.
• Coordinate patient care with specialists, hospitals, and ancillary healthcare providers.
• Educate patients and families on disease prevention, wellness, and healthy lifestyle choices.
• Maintain accurate and timely documentation within the electronic medical record (EMR) system.
• Participate in quality improvement initiatives and team-based care programs.

Qualifications

• MD or DO degree from an accredited medical school.
• Completion of an accredited Family Medicine Residency Program.
• Board Certified or Board Eligible in Family Medicine.
• Eligible for unrestricted New York medical licensure.
• DEA certification or eligibility.
• Strong clinical, diagnostic, and interpersonal communication skills.
• Commitment to patient-centered and community-focused care.

Preferred Qualifications

• Prior outpatient primary care experience.
• Experience with value-based care and population health initiatives.
• Proficiency with EMR systems.
